Third Party Routine Office Visit Documentation

Daily Visit Notes Reflect Initial Findings, Services Provided, and Treatment Plan

Daily visit notes should reflect initial findingsservices provided, and treatment plan including recommendations for services including Chiropractic Manipulative Treatment (CMT), therapiesprocedures and modalities.

Properly Documenting Chiropractic Office Visits for Insurance Providers

Each service rendered during the office visit must be documented appropriately to prevent third-party payer’s denial of services because of a lack of information that proves medical necessity. Keep in mind, progress in resolving the functional deficit shows the chiropractic patient’s progress and is required to tell the story of the medical necessity in the routine office visit.

I am so grateful for Dr. Milo and Dr. Liebing allowing me to the opportunity to attend KMC University's seminar in Denver. Continuing education is so important and Kathy does such a great job at explaining things and helping you through challenges in the office. Her and her staff are so fun to be around and really make the experience great! I highly recommend attending a seminar through KMC University! Thank you Kathy and Staff!
